I referenced "The Linux Bootdisk HOWTO" when I first needed to create custom distributions:

Though the HOWTO was last updated in 2002 its information remains relevant - a statement to how well Unix/Linux knowledge ages. The difference now is the particular files required for your root filesystem, which will be a function of the kernel you build and and your distribution's purpose.
